Turn a vague problem into a useful request
Tell us whether the door moves unevenly, stays closed, reverses during closing or responds inconsistently to a control. Include only observations already made; repeated operation is unnecessary when movement looks unsafe. If an operator display shows a message, record it from a safe position along with the model information you already have.
Track the permitted project through completion
Lake Park’s Community Development guidance points applicants to its CAP system for tracking and inspection scheduling. For a replacement project using that process, identify who receives notifications and who will respond if the reviewer needs clarification. Keep the actual installed product consistent with the final agreed documents rather than treating a substituted door as a purely cosmetic change.

Stop using the door. A responding motor does not make a displaced or uneven assembly safe; report both observations for the assessment.
Who should receive permit notifications?
The project needs a responsible contact who can review and act on them. Agree on that role with the provider so a requested document or inspection is not overlooked.
